Two freight wagons loaded with cement beams which were parked in an area of the train track in San Rafael de Alajuela, detached for reasons still unknown and dragged a vehicle that was crossing the railroad.

According to witnesses the wagons dragged the car for close to 400 meters and eventually overturned crushing the car and killing the driver and passenger; a third victim which was not in the vehicle but apparently a pedestrian at the intersection were the accident happened at the time suffered major injuries and was taken to the hospital in critical condition.

According to Maria Fernanda Arias, spokeswoman for the Costa Rica Train Institute (INCOFER), the INCOFER has been doing work in the area of San Rafael de Alajuela through a hired company and therefore there were several freight wagons in the area, apparently the two wagons were not in movement or use at the time and the movement of the locomotive in a specific maneuver could have caused the wagons to detach this is the preliminary version but they will need to carry out the investigation to discover the reasons why the brake system failed or what could have caused the wagons to move and cause the tragic accident.
